Employer Identification Number Form #: SS-4

An Employer Identification Number (EIN), also known as a Federal Tax Identification Number, is a nine-digit number that the IRS assigns to business entities. The IRS uses this number to identify taxpayers that are required to file various business tax returns. EINs are used by employers, sole proprietors, corporations, partnerships, non-profit organizations, trusts and estates, government agencies, certain individuals and other business entities.


Dependent Care Provider’s Identification and Certification Form #: W-10

If you have enrolled in the Dependent Day Care FSA, you must complete an IRS W-10 form for each day care providing services for which you submit reimbursement requests.


Employee's Withholding Allowance Certificate Form #: W-4

Employees complete Form W-4 and gives it to their employer so their employer withholds the correct Federal income tax from their pay.


Request for Taxpayer Identification Number and Certification Form #: W-9

U.S. persons use Form W-9 to provide their correct taxpayer identification number (TIN) to payers (or brokers) who are required to file information returns with IRS.


Employment Eligibility Verification Form #: I-9

All U.S. employers are responsible for completion and retention of Form I-9 for each individual they hire for employment in the United States. This includes citizens and noncitizens. On the form, the employer must verify the employment eligibility and identity documents presented by the employee and record the document information on the Form I-9.
